To locate the parish registration form, click on the drop down tab under Parish Registration marked "Parish Registration form".  This is a PDF so it will need to be printed for you to fill out the form.  You may return the form by mailing to the parish office, 12210 S. 36th Street, Ste A, Bellevue, NE, 68123, or place in the collection  basket or visit the parish office located on the right as you turn onto St. Matthew campus.

 

You  will receive your welcoming packet  within two weeks.  You will also be contacted  by phone when we have welcoming weekend which is held every other month  to receive your loaf of bread and be welcomed by the parish community.
